Information we collect

If the online appointment-request form is available and you choose to submit it, we collect your name, phone number, selected service, preferred time, language, and technical information needed to protect the form. Please do not submit symptoms or health information through it. While the form is unavailable, the website does not collect these details.

Why we use it

If the form is available, we use the information to receive and respond to appointment requests, prevent spam, maintain security, and meet legal obligations. Submitting the form does not confirm an appointment; staff will contact you to confirm.

Service providers and retention

When the form is enabled, requests are processed through our Cloudflare website and security services and our Resend email provider, with access limited to those who need it. These providers may process information outside Thailand. We retain appointment-request information for no more than 180 days unless a longer period is required by law, then delete or de-identify it as appropriate.

Optional analytics cookies

Google Analytics does not load unless you accept. We use only general page-visit and contact statistics and do not send form entries, phone numbers, service choices, or request IDs. You may reject analytics or change your choice at any time through Cookie settings in the footer. Google may process analytics information outside Thailand.

Your choices and rights

You may contact us by phone or LINE instead of using the form. You may also request access, correction, or deletion of information you submitted, subject to applicable law.
